Long Term Care Ombudsman Champions

Elder abuse takes many forms.  

Imagine being a frail adult on hospice, living in and dependent on the health services of a Skilled Nursing Facility, and being notified by the facility that Medicare will no longer cover the cost of your care and that you must leave.  Where would you go? Who could you turn to? 

Long Term Care Ombudsman help elderly residents in this situation by advocating for residents' rights to ensure that they have the required care in order to live with the dignity and respect they deserve.

Each year, hundreds of thousands of adults over the age of 60 are abused, neglected, or financially exploited. Elder abuse, including neglect and exploitation, is experienced by 1 out of every 10 people, ages 60 and older, who live at home. 

Who We Are:

CCSCC LTC Ombudsmen help advocate for the rights of the most frail seniors and disabled adults living in residential care facilities. The LTC Ombudsmen investigate and resolve complaints of elder abuse in senior living facilities.There are over 360 senior living facilities in Santa Clara County serving 12,000+ seniors and disabled adults and the number of facilities is growing each year. The LTC Ombudsman program consists of 3.5 staff and 48 highly dedicated, certified volunteers to serve the entire county. Your generous donation will help add a staff member to support serving 12,000 seniors and disabled adults in the 13 cities that makeup Santa Clara County.  Your support will help ensure that seniors, disabled adults, and their families have a trained professional to advocate for their rights, investigate and resolve elder abuse complaints, and educate the community at large to identify and report elder abuse.
